FOR PATIENTS & FAMILIES

A clear path
to care at home.

Whether you’re planning after a hospital stay or exploring support for an ongoing need, start by talking with your provider and our office.

  1. 01

    Start with a call

Tell our office you’re exploring home health. Ask about your location, the services you need, and the referral process.

  2. 02

    Talk with your provider

Your physician or other authorized provider determines whether home health fits your needs and orders appropriate care.

  3. 03

    Understand your plan

The agency reviews the referral, care needs, service availability, and coverage before arranging care.

Before your first conversation.

You don’t need to have the process figured out. These details can help the office answer initial questions:

  • The location where care would be provided.
  • The name of the physician or provider involved in care.
  • Your health plan and questions about benefits.
  • Any planned discharge date or referral you want to discuss.

Ask how to share referral documents or medical records through the agency’s appropriate channel.

Understanding the first visit.

After a referral is accepted, the agency evaluates the patient’s needs and works with the ordering provider on the care plan. Ask who will visit, what the visit will involve, and how scheduling and follow-up will work.

Questions worth asking.

LET’S TALK ABOUT THE NEXT STEP
